City of Kingston Arts Fund - Grant Reports

CKAF has changed our final reporting requirements in order to collect information that can be used to produce a CKAF Impact Report. The changes for each grant program are listed below.

Grant recipients are required to complete interim and final reports, however, answering these additional questions are optional for 2009 Grant Recipients. The additional questions will become mandatory for 2010 Grant Recipients.

Project Report Requirements:

The interim Project Grant report for the City of Kingston Arts Fund should provide a typed half-page to one-page (maximum) narrative status report providing:

• Title and short description/outline of Project
• Confirmation that the Project will take place and outline of any substantive changes to the Project since your application
• Short outline of current Project schedule and dates of Project events(s) – is it currently progressing on budget and as planned?
• Outline of any additional funding that has been secured for the Project.
• Confirmation of the elements of the Project that are in place at this time (if any).

2011 Interim Reports should be submitted as a text document attachment via email to grants@artskingston.com no later than December 31st, 2011.

Operating Report Requirements:

The interim Operating Grant report for the City of Kingston Arts Fund should provide a typed half-page to one-page (maximum) narrative status report providing:

• Name of Organization in receipt of the Operating Grant.
• Brief outline of any substantive changes to the Operations & Activities outlined in your original application.
• A brief statement regarding the status of your organization’s plans for the current fiscal year. Are your organization’s activities currently progressing on budget/as planned?
• Outline of any additional funding that has been secured for the Organization since your application was originally submitted.

The Report should be submitted as a text document attachment via email to grants@artskingston.com no later than 31 December 2010.

Final reports for Operating Grants are included in the following year’s application form.

Please note: If your project pays fees to any participant (more than $500 in a year), the Canara Revenue Agency (CRA) may require that you issue T4 forms (salaries) or T4A forms (fees) at year end for tax purposes. For more information on payroll, taxes, T4 and T4A forms please contact the Canada Revenue Agency (CRA)  http://www.cra-arc.gc.ca.
